Gateway Medical Associates, P.C. is a
physician owned and governed, fully integrated group of
primary care physicians and allied health providers who
deliver quality, cost-efficient medical care with the
capability of managed care contracting, including global
payment and capitation.
The founding members hold a shared commitment to developing
and being employed through a physician led medical practice.
Therefore, Gateway Medical Associates is designed to be a
physician led and dominated medical group. Its goal is to
develop and maintain a sufficient market presence in its
patient catchment area to give its members control over their
own destiny.
Gateway Vision
To create a fully integrated primary
care group that provides the leadership and excellence in the
delivery of health care to the Chester County community. We
will develop the ability to prudently apply resources allowing
for the community to obtain a wide ranging continuum of health
care, being responsible clinically and fiscally for the health
status of the population we serve. We will strive as well for
a practice environment that is professionally and personally
satisfying.
Appointments:
Patients are seen by appointment
only, and we request that you call in advance so that we may
reserve a time for you. Same day appointments are available.
In emergency situations, immediate appointments may be
arranged. We truly believe that your time is valuable, and
make every effort to stay on schedule. However, on some
occasions certain medical situations or emergencies will take
precedence. We ask for your understanding in these situations.
Similarly, if you should need to cancel an appointment, please
call us at least 24 hours in advance.
All patients are expected to arrive on time for their
scheduled appointment. New patients are asked to arrive 10
minutes prior to their scheduled time.
Rescheduling of
Appointments
Please call the phone receptionist as
soon as possible to arrange a new appointment.
Cancellations:
We request you notify us at least 24
hours prior to the scheduled appointment time.
Referrals:
The referral office is open Monday
through Friday from 8:00 a.m. until 4:30 p.m. to address your
referral needs. Please allow ample time for your referral to
be processed prior to your appointment with your specialist.
To request a referral, please call your office. Please be
prepared to give your name, phone number, name of specialist
and reason for visit to specialist. Some referrals can now be
done electronically (i.e. Aetna US Healthcare) and do not
require you to take anything with you to your specialist. Any
referrals that cannot be done electronically (i.e. Keystone
Health Plan East) can be mailed to your home for your
convenience. If you want your referral mailed to your home,
please allow time for the postal service to deliver it to you
before your appointment with the specialist. You must request
your referral to be mailed to you otherwise it will be
available for pick-up at our office during regular business
hours. We do not process referrals on weekends or holidays,
please keep this in mind when making your request.
Prescription
Refills:
You have the following options to
obtain a refill on you medication.
- You may call our office directly.
- You may have your pharmacy call us for your prescription
refills.
You will need to know the name and dosage of the medication,
the pharmacy name and phone number (this is all on the
prescription bottle). We will provide written prescriptions
when necessary: they will be mailed to patients home or
picked up at our office. Please call the office one week prior
to the end of your medication. Refills are called to the
pharmacy by the end of the next business day. Please call your
pharmacy to make sure your prescription has been called in and
is ready to be picked up prior to going to the pharmacy. We
will call you only if we are unable to refill your
prescription or need additional information regarding the
refill.
Insurance Billing:
We will submit claims to all primary,
secondary and tertiary insurance carriers with whom we
participate. The patient must present their valid insurance
card upon registration for the appointment. Patients are
ultimately responsible for charges incurred at the office.
Payment for Medical Services
Co-payment, non-covered services, and
any previous balances are due at time of service. Patients who
anticipate difficulty in paying for medical services are
encouraged to contact our billing department at 610 594-9015
prior to your next visit.
TYPES OF PAYMENT:
We accept cash, personal checks,
Visa, Mastercard and debit cards. Payment is expected at the
time of service.
SELF-PAY PATIENTS:
Payment is expected in full at the
time of service. If a patient requests and requires a payment
plan, arrangements will be made prior to the time of service
and finalized at the time of check in.
Medical Records:
Patient medical records are
confidential. No information can be released without a signed
consent by the patient. Please be aware it may take up to two
weeks to process your request.
