Prescription Refills

Refills are ideally done at the time of your office visit

You can call your office to request a refill. When calling you will need the following information:

Your name and date of birth
Name & dosage of the medication
Pharmacy name & phone number
 

IMPORTANT REMINDERS REGARDING PRESCRIPTION REFILLS

Call the office one week prior to the end of your medication when possible
Prior to going to the pharmacy, please call you pharmacy to make sure your prescription is ready for pick-up
The office will call you only if we are unable to refill your prescription or need additional   information regarding the refill.
Under certian circumstances it may be required that you be seen by a physician prior to refilling your prescription
